GRAND RAPIDS (WOOD-AM) - Ascension Healthcare says some Michigan customers may have had their name, address, phone number, email address, social security number and other personal information exposed.
The issue stems from a data breach in December.
They say it's due to third-party software and a business partner.
Ascension says it will offer two years of credit monitoring and identity theft protection to those who have been impacted.
Ascension operates the former Borgess hospitals.
